ABSTRACT

Objective:To study the clinical effects of Yishen-Shengxue Decoction on the prevention and treatment of bone marrow suppression in acute myeloid leukemia after chemotherapy. Methods:A total of 60 patients in Beijing Longfu Hospital from June 2018 to March 2020 were randomly divided into control group and observation group, 30 cases in each group. The control group was treated with western medicine and blood transfusion, while the observation group was treated with Yishen-Shengxue Decoction on the basis of the control group. Both groups were treated for 2 weeks. We recorded the occurrence time and duration of Ⅳ degree myelosuppression of the two groups, compared the score of symptoms and signs, injection volume of recombinant human granulocyte colony stimulating factor (G-CSF) and blood transfusion volume, and recorded the incidence of adverse reactions after chemotherapy. Results:The occurrence of myelosuppression in the observation group was significantly later than that of the control group (5.07 ± 0.87 d vs. 3.83 ± 1.15 d; t=4.695, P<0.01), and the duration of grade Ⅳ myelosuppression was significantly shorter than that of the control group (7.20 ± 0.76 d vs. 10.03 ± 1.30 d; t=10.305, P<0.01); The quantity of granulocyte colony stimulating factor injection in the observation group was significantly less than that of the control group (7.2 ± 0.8 vs. 10.0 ± 1.3, t=10.305), and the quantity of red blood cell suspension (2.5 ± 1.5 U vs. 4.7 ± 1.5 U, t=7.749) and platelet transfusion (1.7 ± 0.5 U vs. 3.1 ± 0.9 U, t=5.879) were significantly less than that of the control group ( P<0.01); the quantitative score of symptoms and signs in the observation group were significantly lower than thoseof the control group ( t values were 18.208, 15.129, respectively, all Ps<0.01). The adverse reactions of the patients in the observation group after chemotherapy were significantly less than those of the control group, and the incidence of infection, bleeding and ECG abnormalities were statistically significant ( χ2 values were 7.500, 10.000, 4.286, respectively, all Ps<0.01). Conclusion:Yishen-Shengxue Decoction is helpful to delay the occurrence of myelosuppression, and promote its recovery, reduce various adverse reactions during myelosuppression, and improve the quality of life.

ABSTRACT

Objective:To observe the clinical efficacy of Ziyin-Heyang Decoction on the treatment of glucocorticoid-dependent primary immune thrombocytopenia. Methods:A total of 70 patients with ITP in Xiyuan Hospital of China Academy of Chinese Medical Sciences from November 2015 to November 2017 who met the inclusion criteria were divided into two groups according to the random number table method. 33 cases in the treatment group and 32 cases in the control group completed the reaserch. The control group received standard dose of prednisone acetate orally, and the dose was gradually reduced according to platelet count; and the treatment group was given Ziyin-Heyang Decoction on the basis of the control group. Both groups had one course of treatment for 8 weeks, with a total of 2 courses of treatment. Before and after treatment, TCM syndrome score was performed, PLT was detected by impedance method, bleeding score was calculated, daily dosage of prednisone acetate tablets was recorded, and platelet related antibodies (PAIgA, PAIgG, PAIgM, PAIgD) were detected by flow cytometry. Then clinical and syndrome efficacy were evaluated. Results:The total effective rate was 75.8% (25/33) in the treatment group and 53.1% (17/32) in the control group, and the difference in efficacy was statistically significant ( Z=2.846, P<0.05). For the TCM syndromes, the total effective rate was 100.0% (33/33) in the treatment group and 75.0% (24/32) in the control group, and there was a statistical difference between them ( Z=3.841, P<0.05). After treatment, the platelet count improvement [(97.6 ± 39.9)×10 9/L vs. (70.9 ± 16.6)×10 9/L, t=3.540] of the treatment group was significantly higher than that of the control group ( P<0.05). The TCM syndrome score [6.00 (4.00) vs. 10.00 (6.00), Z=3.510] of the treatment group was significantly lower than that of the control group ( P<0.05). After the treatment, the dose of prednisone acetate tablets [5.00 (17.50) mg/d vs. 27.66 (11.98) mg/d, Z=4.186] of the treatment group was significantly lower than that of the control group ( P<0.05). Bleeding score decreased in both groups, but there was no significant difference between them ( P>0.05). There was no statistically significant change in platelet related antibody levels in the treatment group before and after treatment ( P>0.05). Conclusions:The Ziyin-Heyang Decoction has advantages in improving platelet count, with drawing glucocorticoid and improving symptoms.

Objective:To investigate the relationship between TP53 gene mutation and overall survival (OS) time of myelodysplastic syndrome (MDS).Methods:Databases, including PubMed, Cochrane Library and Embase were searched for relevant studies published up to 20 October, 2019. The corresponding hazard ratio ( HR) and their 95% confidence interval ( CI) for OS from multivariate Cox proportional hazard models were extracted. The combined HR with their 95% CI was calculated by using fixed or random effect models. Meta-analysis was performed by using Revman 5.3 software. Results:A total of 1 033 patients from 6 studies were enrolled. Meta-analysis results showed that OS time in TP53 gene mutation group was shorter than that in wild type group for patients with MDS ( HR = 2.16, 95% CI 1.52-3.07, P < 0.01). The prognostic risk for post-transplantation patients with MDS ( HR = 2.69, 95% CI 1.63-4.43, P < 0.01) was lower compared with that for patients treated by azacitidine( HR = 2.89, 95% CI 1.37-6.08, P = 0.005). Conclusion:TP53 gene mutation is a risk factor affecting OS of MDS patients.

ABSTRACT

The Chinese national college students' life science competition has been held for three times, with good organization, large scale and high participation degree. The competition plays an important role in promoting life science education and research. This paper reports the form and status of the competition, statistically analyses the registration data and competition results by region and year, based on the previous three competitions. By combining new changes and understanding in the field of life science, we also indicate prospects on how to better promote the competition.

ABSTRACT

This study intends to obtain recombinant proteins of ALT1 and ALT2 isozymes by using genetic recombination technology. Monoclonal antibodies ALT1 and ALT2 with high specificity and high activity were prepared and screened (ALT1 monoclonal antibody has been successfully prepared and published). The localization, distribution and expression of ALT1 and ALT2 isozymes in human tissues were discussed. The ALT2 genes were amplified from human liver cancer cell (HepG2) by RT-PCR method. The mature ALT2 gene was subcloned into the pET32a-ALT2 prokaryotic expression vector. Its ligation product was transformed into BL21(DE3) competent cells, and transformed into competent cells to express ALT2 proteins induced by IPTG. The recombinant proteins of ALT2 were purified by nickel column (Ni⁺) affinity chromatography. Balb/c mice were immunized with recombinant proteins of ALT2. Positive serum mouse spleen cells and myeloma cells SP2/0 were selected for cell fusion. The positive cell lines were selected by indirect ELISA and subcloned by limited dilution method. Affinity chromatography was used to purify ALT2 antibodies. The expression and distribution of ALT2 in human normal tissues were detected by RT-PCR and Western blotting. Results show that the expression of ALT isoenzyme in tissues was almost the same at gene mRNA level and protein level. ALT1 is highly expressed in liver, kidney and skeletal muscle, and moderately expressed in gastrointestinal smooth muscle. ALT2 is highly expressed in fat, skeletal muscle and myocardium, and is poorly expressed in gastrointestinal smooth muscle. Immunohistochemical studies show that ALT1 is highly expressed in hepatocytes, renal medullary tubules and muscle fibers, ALT2 is highly expressed in adipocytes and myocardial cells, and ALT1 and ALT2 in gastrointestinal tissues are mainly expressed in mucosa of upper intestinal wall region. The results showed that the isoenzymes ALT1 and ALT2 were mainly expressed in the mucosa of the upper part of the intestinal wall. It is widely distributed in the tissues, providing theoretical basis for understanding the mechanism of ALT activity increase under different pathological conditions.

Objective@#To investigate the clinical effects of Qinghuang powder, low-intensity chemotherapy and Qinghuang powder alternated with low-intensity chemotherapy in the treatment of elderly acute myeloid leukemia patients progressed from myelodysplastic syndromes (MDS-AML).@*Methods@#A total of 32 elderly patients with MDS-AML treated in Xiyuan Hospital, China Academy of Chinese Medical Sciences from January 2014 to December 2017 were retrospectively analyzed. Of them, 12 patients received Qinghuang powder (Qinghuang powder group), 6 patients received Qinghuang powder alternated with low-intensity chemotherapy (alternating group), and 14 patients received low-intensity chemotherapy (low-intensity chemotherapy group), based on the real world of patient's voluntary choice of treatment. The efficacy and adverse reactions in the 3 groups were observed.@*Results@#The overall response number of Qinghuang powder, alternating, and low-intensity chemotherapy groups were 2, 2 and 4 cases, respectively, there was no significant difference among the 3 groups (P > 0.05). The median overall survival time of 3 groups were 14, 12 and 8 months, respectively, there was no significant difference among the 3 groups (P > 0.05). Two cases in Qinghuang powder group presented with gastrointestinal reactions; 3 cases in alternating group with myelosuppression, 2 cases with liver function damage, and 2 cases with gastrointestinal reaction; 11 cases in low-intensity chemotherapy group presented with bone marrow suppression, 3 cases with liver function damage, 7 cases with gastrointestinal reactions, and 2 cases with myocardial enzyme changes. The incidences of myelosuppression, liver function damage and myocardial enzyme changes in Qinghuang powder group were lower than those in the alternating group and low-intensity chemotherapy group.@*Conclusion@#The efficacy of Qinghuang powder or Qinghuang powder alternated with low-intensity chemotherapy for the treatment of elderly MDS-AML is not worse than the low-intensity chemotherapy, furthermore Qinghuang powder or Qinghuang powder alternated with low-intensity chemotherapy has fewer adverse reactions and better tolerance.

ABSTRACT

Objective@#To analyze the gene mutations in the patients with myelodysplastic syndromes (MDS).@*Methods@#Forty-seven patients with MDS newly diagnosed in Xiyuan Hospital, China Academy of Chinese Medical Sciences from January 2016 to July 2017 were enrolled. NGS 127-gene panel was used to detect the gene mutations, and the relationship between the gene mutations and the clinicopathological features was also analyzed.@*Results@#Thirty-one (66.0 %) cases had gene mutations in 47 patients with MDS, and 23 gene mutations were detected with clinical significances. There were 7 mutant genes with a mutation frequency over 5 % in the population, including U2AF1 (23.4 %), SF3B1 (12.8 %), ASXL1 (10.6 %), TET2 (8.5 %), BCOR (8.5 %), TP53 (8.5 %) and DNMT3A (6.4 %) in turn. Among 31 patients with gene mutations, 16 (51.6 %) patients had ≥ 2 synergistic mutations, and 12 cases had synergistic mutations in different genetic functional groups, which was higher than that in same genetic functional groups (4 cases). There was a tendency of coexistence in IDH2-KRAS, IDH2-SRSF2, IDH2-STAG2, KRAS-SRSF2, KRAS-STAG2, RUNX1-PHF6, EZH2-ASXL1, EZH2-ZRSR2, and NPM1-NRAS (all P < 0.05). The variant allele frequency (VAF) of signaling pathway related genes including JAK2, KRAS, NRAS, SH2B3 was low in general and in a sub-clone status. JAK2 gene mutation was observed in 1 case with MDS-U. SH2B3 gene mutation was observed in a patient with very poor prognosis of karyotype. SETPB1 and EZH1 gene mutations were observed in two patients with high-risk revised international prognostic scoring system (IPSS-R).@*Conclusions@#The common mutated genes include U2AF1, SF3B1, ASXL1 and TET2. The genes in different genetic functional groups tend to synergistic mutations. Gene mutations can be used to predict the prognosis of diseases and become the target in the treatment of MDS.

ABSTRACT

Phenol-soluble modulins (PSMs) are a novel family of small amphipathic, alpha-heli-cal peptides with strong surfactant-like properties. PSMs have multiple roles in staphylococcal pathogenesis and are considered as important virulence-associated factors. They may cause lysis of many eukaryotic cells, such as human neutrophils,erythrocytes and dendritic cells;induce the expression of pro-inflammatory cyto-kines;facilitate the structuring and detachment of biofilms;influence the expression of other genes. This re-view summarizes the classification,structure,regulatory effect on gene expression and biological function of PSMs. The potential avenues to target PSMs for drug development against staphylococcal infections are also evaluated in this paper.

ABSTRACT

Objective To investigate the survival of oral arsenic-containing Qinghuang Powder (QHP) and low intensive chemotherapy (LIC) in the treatment of elderly patients with acute myeloid leukemia (AML).Methods Forty-two AML patients older than 60 years in Xiyuan Hospital from January 2015 to December 2017 were retrospectively analyzed.Of them,20 cases were treated with QHP (QHP group),22 cases were treated with LIC (LIC group).The survivals of the two groups were compared.Results There was no significant difference of median survival time (13 months vs.13.5 months,x2 =0.096,P =0.757),1-year survival rates (59.1% vs.70.0 %,x2 =0.543,P =0.461),2-year survival rates (13.6 % vs.15.0 %,x2 =0.016,P > 0.05),and 3-year survival rates (4.6 % vs.5.0 %,x2 =0.005,P > 0.05) between LIC and QHP groups.There was no significant difference of median survival time in age ≥75 year (12 months vs.12.5 months,x2 =1.317,P =0.251),performance status scores > 2 (12 months vs.12 months,x2 =0.834,P =0.361),hematopoietic stem cell transplantation with combined disease index > 2 (12 months vs.13 months,x2 =1.726,P =0.189),secondary AML (10 months vs.14 months,x2 =1.552,P =0.213),and poor cytogenetics (12 months vs.8 months,x2 =0.479,P =0.489) between LIC and QHP group.Conclusion The survival of elderly AML patients is considerable in patients treated with oral QHP and LIC,which suggests that oral QHP may be an equivalent alternative treatment since elderly AML (especially more than 75 years) patients refused to LIC therapy.

ABSTRACT

Synthetic biology is a fast moving interdisciplinary branch of biology and engineering. To educate the next generation of synthetic biology scientists, the International Genetically Engineered Machine (iGEM) competition was established. In the past eleven years, many Chinese teams have participated in this event, but no thorough review and analysis have been carried out. In this paper, we collected the data and information of the Chinese teams from the iGEM website and analyzed the number, distribution and performance of Chinese teams in iGEM competition. We also described contributions made by the Conference of China iGEMer Community (CCiC) organization. The contributions to China higher education made by the iGEM competition were also summarized. Finally, we proposed several suggestions for the development of the iGEM competition in China. We envision the iGEM competition will continue to promote the innovative education and cultivation of the next-generation synthetic biology scientists in China.

ABSTRACT

Objective To analyze the clinical efficacy and safety of compound Qinghuang powder (compound QHP) for treatment of myelodysplastic syndromes (MDS) and its association with blood arsenic concentration (BAC). Methods 40 patients with MDS were treated with compound QHP, and the clinical efficacy, safety, and its association with BAC were evaluated after treatment for 6, 9 months, respectively. Results After treatment for 6 months, the rate of hematology improvement was 32.5 % (13/40), and the effective rate was 87.5%(35/40). 21 cases depended on the blood transfusion before treatment, after treatment 6 cases completely got rid of blood transfusion and the blood transfusion of another 6 cases was decreased by more than 50 %. The absolute neutrophil count was increased from (0.50±0.13)×109/L to (0.93±0.33)×109/L (t= 4.130, P= 0.0008). The hemoglobin content was increased from (71.06±14.82) g/L to (80.41±27.35) g/L (t= 2.233, P= 0.0321). After treatment for 9 months, 76.2 % (16/40) of the patients got rid of blood transfusion or blood transfusion reduction was more than 50%. The platelet count was increased from (45.04 ± 24.38)×109/L to (60.65±29.46)×109/L (t= 2.241, P= 0.0335). The incidence of abdominal pain and diarrhea after treatment for 1, 3 and 6 months were 12.5 % (5/40), 10.0 % (4/40) and 5.0 % (2/40), respectively, all belonging to mild level . Before treatment , there were 12 patients with abnormal liver function , including 6 cases back to normal after treatment, and 6 cases of significantly relieved, without new case with abnormal liver function. Before treatment, there were 10 cases with abnormal myocardial enzymes, including 1 cases back to normal after treatment and 9 cases significantly relieved, without new case with abnormal myocardial enzymes. No patient with abnormal renal function was observed before and after treatment. The BAC was (7.71±5.65) μg/L before treatment, which was significantly lower than that of 1, 3 and 6 months [(29.27±9.07)μg/L, (27.79 ±10.18) μg/L and (31.98 ±12.55) μg/L respectively, all P 0.05). The BAC in efficacy group [(33.48 ±12.56) μg/L] was significantly higher than that in non-efficacy group [(21.46 ±6.00) μg/L] (t=2.089, P=0.035). 12.5% (5/40) of the patients had mild gastrointestinal side effects after treatment for 1 month, while the BAC of them [(16.93 ±1.80) μg/L] was significantly lower than that in patients without gastrointestinal side effects [(31.78±1.39 ) μg/L, P<0.0001]. The occurrence rate of abdominal pain and diarrhea was decreased after treatment for 3 and 6 months, while the BAC was increased gradually. Conclusions Compound QHP is effective in the treatment of MDS with mild adverse reactions. There is no damage to the heart, liver, and renal function. Besides, it shows that reducing the gastrointestinal adverse reactions and maintaining the effective concentration of BAC play a significant role in the effect of compound QHP in the treatment of MDS.

Medical microbiology is an important basic subject in medical colleges.In view of the multifarious and abstract nature of the specialized course and the dull atmosphere in classroom teaching,we have adopted a doggerel teaching method in order to enliven the classroom atmosphere and improve the class teaching quality.After long-term teaching practice,it's been proved that the proper use of doggerel in classroom teaching can not only refresh boring theory and enhance students' learning interesting,but also simplify the complicated context and improve their memorizing ability and learning efficiency,posing a significant impact on the teaching effects of medical microbiology.

Objective To search for application ways for the safe and effective clinical methods of arsenic-containing Compound Qinghuang Powder (Compound QHP) for the treatment of myelodysplastic syndrome (MDS). Methods Totally 200 patients with MDS were included in the study and treated with Compound QHP. After one-month treatment, the 60 patients with the blood arsenic concentrations 0.05). Conclusion In application of Compound QHP, the blood arsenic concentration can be monitored to adjust the daily dose of realgar, thus to increase the effective blood arsenic concentration, and then improving efficacy without increasing the clinical toxicity.

Purpose To observe the thrombolytic effect of different ultrasonic frequencies combined with urokinase-containing targeted microbubble contrast agent on rabbit femoral artery,to explore the main influencing factors,and to determine the potential indicators related to recurrent embolism in the microcirculation.Materials and Methods Unilateral femoral arterial thrombosis models were established in the selected 72 New Zealand white rabbits,which were randomly divided into 12 groups,6 rabbits in each group.This study was performed on an experimental combination of three factors and different levels,including different ultrasonic frequencies (1.6,2.2,2.8 MHz),different ultrasonic irradiation time (30 and 60 min),and different urokinase dose (3 and 6 mg).Thrombolysis with urokinase-containing targeted microbubble was performed under low frequency ultrasonic assisted irradiation,the recanalization status of blood vessels was observed,and recurrent embolism in the microcirculation was confirmed by HE staining.Furthermore,rabbit blood samples were collected,with indicators,including 6-keto-prostaglandin F1a (6-keto-PGF1a),Thromboxane B2 (TXB2),P/T ratio (6-keto-PGF1a/TXB2) and P-selectin (SP) being detected.Results All the vessels were recanalized.There was no occurrence of recurrent embolism in the group with ultrasonic frequency of 2.2 MHz,radiation time of 30 min,and urokinase dose of 3 mg.Rabbits' blood vessels were observed to be not completely recanalized in other groups,accompanied with different degree of recurrent embolism in the microcirculation.6-keto-PGF 1 a content of the rabbits in the group without recurrent embolism obviously increased after thrombolysis,and the difference was statistically significant (P<0.05).While,there was no statistical difference in other indicators (P>0.05).Conclsion The thrombolysis with ultrasound frequency of 2.2 MHz,irradiation time of 30 min,and urokinase dose of 3 mg could achieve complete recanalization of blood vessels.Under the certain conditions of ultrasonic frequency,irradiation time and urokinase dosage,thrombus can be effectively dissolved.However,there may be risk of recurrent embolism in the microcirculation during thrombolysis process.The increase of 6-keto-PGF 1a content has a certain effect on reducing the formation of recurrent embolism in the microcirculation.

ABSTRACT

Clonal progression to myelodysplastic syndromes (MDS) or acute myeloid leukemia (AML) remains a dreaded complication for a subset of patients with bone marrow failure (BMF). Recognizing risk factors for the development of MDS or AML would inform individualized treatment decisions and identify patients who may benefit from early or upfront hematopoietic stem cell transplantation. Now that next-generation DNA sequencing is available in the clinical laboratory, research has focused on the implications of germline and somatic mutations for diagnosing and monitoring patients with BMF. Most germline genetic BMF disorders are characterized by a high propensity to develop MDS or AML. Recent studies of somatic variants in marrow failure revealed a high frequency of clonal hematopoiesis with the acquisition of mutations in genes associated with MDS or AML. The evaluation and implications of germline and somatic mutations for the development of clonal disorders in patients with BMF and challenges of clinical genetic testing will be summarized in this paper based on the reports from the 58th American Society of Hematology (ASH) Annual Meeting.

ABSTRACT

Objective To explore the influence of genetic and environmental factors on adolescents depressive symptoms. Methods The depressive mood were investigated in 107 twin pairs from Chongqing (aged from 11 to 18 years old) using Beck Depression Inventory (BDI). The Parenting Styles and Dimensions Questionaire (PSDQ), Family Stresses Questionnaire (FSQ), Family Adaptability and Cohesion Scale 2nd Edition Chinese Version (FACES II-CV) and Stressful Life Event (SLE) were applied to assess their environment factors. The blood samples were collected to identify zygosity of twins. Structural equation model was performed to evaluate the effects of the additive genetic factors (A), shared envi?ronment factors (C) and individual specific environmental factors (E) on the adolescents depressive mood. Results The scores of depressive mood were negatively correlated with father’s education level (r=-0.15, P=0.03), mother’s educa?tion level (r=-0.17, P=0.01), scores of the authoritative-parenting-style (r=-0.18, P0.05). The effects of A and E on adolescents depressive symptoms were 0.37 (95%CI:0.14~0.57) and 0.63 (95%CI:0.43~0.86), respectively. Conclusion The authoritative-parenting-style and fami?ly adaptability are negatively correlated with depressive mood. Family stresses are positively correlated with depressive mood. Adolescents depressive mood is influenced by genetic factors and, to a greater extent, specific environmental factors.

Presently, there are many issues in traditional Chinese medicine (TCM) decoction, such as the uncertain sources of TCM, the lack of reminder for medication taboo, the nonstardard herb operation, and difficult supervision, etc. A digital service system of TCM decoction was established to solve the problems mentioned above. The digital service system mainly includes automatic coding for checking in & out, drug medication taboo database, digital operation in decoction, distribution through 2D code, the corresponding application for mobile phone, and the information supervision platform for TCM decoction. The digital service system of TCM decoction can track the quality & duty of the pieces, remind decoction medicine contraindications, improve the standard operation process of decoction, develop decoction distribution & tracking through cell phone, save the waiting time, and hence provides a new supervising method for TCM decoction. The digital service system of TCM decoction solves the key issues for the formula, operation, delivery and supervision of TCM. In the same vein, this system will expand the market share of TCM decoction and promote the development of TCM.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the influence of genetic and environmental factors on the executive function of twin children.</p><p><b>METHODS</b>The executive function of 122 twin pairs from Chongqing (aged from 6 to 18 years) were investigated with the perseverative errors of the Wisconsin Card Sorting Test. Structural equation modeling was performed to evaluate the effects of the additive genetic factors (A), dominance genetic latent factors (D) and individual specific environmental factors (E) on the executive functions.</p><p><b>RESULTS</b>The effects of D and E on perseverative errors were 0. 77 (95%CI: 0.65-0.84) and 0.23 (95%CI: 0.16-0.35), respectively. The probability of perseverative errors showed a significant negative correlation with family functioning and with the total GHQ-12 scores of the fathers and mothers (r:-0.335, -0.335, and -0.219, respectively, P<0.01). Perseverative errors were not significantly correlated with the parenting styles and stress life events.</p><p><b>CONCLUSION</b>Perseverative errors are influenced by a common dominance genetic latent factor and individual specific environmental factors, but the role of environmental factors such as family functioning and parental health can not be ignored.</p>
